2. Enter the Pin# 1211, and then enter your refill information. Your refill request will be sent directly to or pharmacy system, and will be ready to pick up when you arrive. Print off this coupon, and bring it to the store with you.

*Only one coupon per patient can be used. This coupon is not valid, unless used with the Rx-Refill System. Must bring coupon with you when picking up your prescription.
